Ice-Man (Cop): Killed in action.
Dragon-Man Chan (NPC Netrunner): Died of invasive brian surgery after Security believed he had been compromised by Rache Bartmose.
? (super-strong Solo played by Paul, introduced in same adventure as Bane): Shot through eye by Euro-Solo.
Widow (Solo): Executed as a "security risk".
Icarus (Runner): Made a scape-goat for failure of mission in LA, executed.
Soverign (Solo): Executed after disasterous mission to San Fransisco.
Viper (Assasin): See "Soverign" above.
Bane (Solo): See "Soverign" above.
Zero (Covert Op): Punched in head by cyber-enhanced bare-knuckles boxing champion.
Toni "Mr. Minago" Minago: Sudden eye infection.

Can we ammend Bane's reason to dying to something like: dead due to some other muppet on the mission showing their Arasaka badge to an npc whilst undercover?

Care to explain that one, or don't I want to know?
